He was diagnosed with Hemophagocytic Lymphohistiocytosis (HLH), a rare but potentially fatal immune condition driven by extreme inflammation.<\/p>\n<p>Zadran has been reported to be dealing with stage four HLH, which can seriously disrupt how the body regulates inflammation and may harm key organs including the bone marrow, liver, spleen, and lymph nodes. The situation is especially worrying given that HLH can quickly weaken immunity and trigger complications from infections.<\/p>\n<h3>Quick facts<\/h3>\n<ul>\n<li>Shapoor Zadran is in ICU care in New Delhi after an HLH diagnosis.<\/li>\n<li>The reported diagnosis is stage four HLH, described as a severe hyper-inflammation condition.<\/li>\n<li>He played 44 ODI matches and 36 T20Is for Afghanistan from 2009 to 2020.<\/li>\n<li>His brother Ghamai Zadran said symptoms began in October last year.<\/li>\n<li>Zadran was admitted to a New Delhi hospital on January 18.<\/li>\n<li>A bone marrow test in late March confirmed stage four HLH.<\/li>\n<li>Recurring infections included dengue, alongside a major fall in red blood cell counts.<\/li>\n<\/ul>\n<p>In an update shared by his younger brother, Ghamai Zadran, the health problems began in October of last year. He says doctors advised Shapoor to get medical treatment in India, setting in motion efforts to move him quickly for care.<\/p>\n<p>Ghamai also explained that help came from within Afghanistan cricket circles. With support linked to Rashid Khan and the Afghanistan Cricket Board (ACB), the visa process was reportedly sped up, enabling Zadran to reach a New Delhi hospital for admission on January 18.<\/p>\n<h3>What the scans showed<\/h3>\n<p>Ghamai described the illness as unusually severe from the start. He said it was not limited to one area of the body, and that the infection affected him broadly, including tuberculosis (TB), which he added had even spread to the brain. The information, he said, was identified through MRI and CT scans.<\/p>\n<p>As the situation unfolded, the cricket community began rallying around Zadran and his family. The ACB chairman Mirwais Ashraf reportedly raised the case with ICC chairman Jay Shah, while Rashid Khan also reached out through IPL-linked ties, including his Gujarat Titans connection.<\/p>\n<p>Rashid is also said to have visited Zadran during Afghanistan\u2019s Men\u2019s T20 World Cup match in New Delhi earlier this year. The support continued later as well, with Afghanistan spinner AM Ghazanfar\u2014currently part of the Mumbai Indians squad for IPL 2026\u2014visiting on April 3 ahead of his side\u2019s fixture against Delhi Capitals.<\/p>\n<p>Former Afghanistan captain Asghar Afghan has similarly been involved in assisting the family, travelling between Dubai and Delhi during the treatment period. The close involvement underlines how much Zadran\u2019s role in Afghanistan cricket remains recognized beyond the playing years.<\/p>\n<h3>Condition fluctuates amid infections<\/h3>\n<p>While there were early signs of improvement, the recovery did not hold. Ghamai said Zadran\u2019s condition worsened as infections returned, and this included dengue alongside other complications.<\/p>\n<p>He also pointed to a sharp decline in red blood cell count, which he said further weakened Zadran\u2019s immunity. In such a weakened state, infections can take hold more easily and can make HLH management significantly more difficult.<\/p>\n<p>A bone marrow assessment carried out in late March confirmed stage four HLH, according to the family\u2019s account. Ghamai further said Zadran had been feeling better for roughly twenty days before the infection resurfaced, bringing fever once again.<\/p>\n<p>He added that the illness later tested positive for dengue. In Ghamai\u2019s words, Zadran\u2019s immune system is extremely weak, and the drop in red blood cells has been substantial\u2014factors that have kept the prognosis guarded.<\/p>\n<h3>Hope remains, but with caution<\/h3>\n<p>Despite the setbacks, there is still some cautious optimism. Ghamai said the family is hopeful that improvement will come gradually, and he pointed to the effect of recently administered steroids as a positive sign.<\/p>\n<p>He said the steroids appear to be working, which has given them some hope, even as doctors manage recurring risks from infections.
